Changing a Prescription Using the Adaptation Form (ON Only)

In Ontario, Pharmacists can make changes to a prescription using their professional judgment by sending the change to the doctor using the Adaptation Form in PharmaClik Rx.

To send an Adaptation Form to a doctor in PharmaClik Rx:

1. Select a prescription that has been adapted from one of the following places:
   • Workbench > Prescriptions > To Do.
   • Select Patient button to search for and select a patient. Select the Profile tab.
2. Once you have selected a prescription, select Rx > Services > Adaptation.
3. Specify the following fields:
   • Reason (select a reason from the dropdown).
   • Info From the Pharmacist (e.g. provide details on the change for the prescription).
4. Select the Print or Fax button. Result: Once the Adaptation Form is printed or faxed, an icon will appear beside the prescription to show it has been adapted.
