Entering Basic Doctor Information

To enter basic Doctor information, do the following:

1. Open a Doctor Folder.

2. Enter in the following information:
   • Last Name:Doctor's last name.
   • First Name: Doctor's first name.
   • Language: Language preference for the Doctor Folder.
   • ReAuth Method: Doctor's preference method for reauthorizing prescriptions.
   • Prescriber Type: Doctor's qualification type.
   • License #: Doctor's license number.
   • Addr Line 1: Doctor's address.
   • Addr Line 2: Additional information about a doctor's address (i.e., box number, etc.)
   • City: Doctor's city.
   • Province: Doctor's province.
   • Post Code: Doctor's postal code.
   • Phone: Doctor's phone number.

Note: Use the Address 1, Address 2, Address 3, Address 4 tabs to enter additional address information for the patient.

3. Select the Save button.
